By submitting this form, I wish for my sons/daughters to be accepted on the courses that I will now apply for, at the above that I will select. If you wish to cancel your child's place on the Soccer Camp please e-mail as soon as possible with your child's details and the course you booked. Your refund will be issued immediately minus a £3 cancellation fee. Cancellations within 5 working days of the course start date will not be eligible for a refund. However credit notes will be issued for future courses. Once payment has been received we will send out a soccer camp confirmation / receipt and the welfare form to the address yo supply, all welfare forms are to be filled out and brought with the child on thew day. The Welfare form is about your child, health issues and your emergency contact details, all forms will be held on a secure database and are treated as confidential. They are never shown to a 3rd party. During training camp it is possible for photographs to be taken of your child for future promotion and training purposes.

1. The Soccer Camp reserves the right to cancel any of its venues with at least 72 hours prior notice, a full refund will be given in these circumstances.
2. Soccer Camp will only issue credit notes for courses cancelled with less then 5 working days of the course commencement date.
3. For Cancellations with more than 5 working days a £3.00 admin fee will be charged. If you have paid by Debit or credit card a Fee of 3.5% will also be charged this is to accomodate the Paypal handling fee.
4. The Soccer Camp does not disclose any information to third parties.
